Hypovolemic Shock on the NCLEX

Recognition, fluid resuscitation, transfusion thresholds.

Hypovolemic shock is volume loss. NCLEX tests recognition and fluid resuscitation.

Causes

Hemorrhage (trauma, GI bleed, postpartum). Severe dehydration (vomiting, diarrhea, burns).

Recognition

Hypotension, tachycardia, weak pulses, cool extremities, decreased urine output, altered mental status.

Treatment

Two large-bore IVs. Crystalloid bolus 30 mL/kg. Reassess. Transfuse if hemorrhagic and Hgb low or volume not responding. Treat source.

Frequently asked

Crystalloid vs colloid?

Crystalloid first-line. Albumin reserved for specific scenarios (post-paracentesis, refractory shock in burns).
