Massive Transfusion: 1:1:1 Ratio on the NCLEX
Massive transfusion protocol, when activate, ratio rationale.
Massive transfusion protocols use 1:1:1 ratio of PRBC:FFP:platelets to mimic whole blood.
When activate
Massive blood loss (>1500 mL trauma, postpartum hemorrhage, GI bleed).
Ratio
1 PRBC : 1 FFP : 1 platelet pack. Replicates whole blood composition.
Adjuncts
Tranexamic acid (TXA) within 3 hours. Calcium replacement (citrate in stored blood binds calcium).
