When to draw, target, nomogram-based adjustment.
Heparin drip titration follows aPTT.
Target
aPTT 60-80 seconds (1.5-2× control).
Adjustment
Per nomogram. Low aPTT → bolus + increase. Therapeutic → continue. High → decrease or hold.
Recheck after change
6 hours after rate change.
