Methotrexate on the NCLEX

Methotrexate: the weekly-not-daily error, folic acid, CBC and liver monitoring, infection and pregnancy warnings.

Methotrexate is a folate antagonist used weekly for autoimmune disease. Daily dosing of the weekly regimen kills patients, and the NCLEX knows it.

Weekly, not daily

For rheumatoid arthritis and psoriasis, methotrexate is a once-weekly drug. Taken daily by mistake, it causes fatal marrow suppression, and this exact error recurs in real incident reports. Safety habits that answer exam questions: the patient names their dose day, prescriptions state the day of the week, and any 'daily methotrexate' order for autoimmune disease gets questioned before it gets given.

Folic acid alongside

As a folate antagonist, methotrexate causes mouth sores, GI upset, and marrow suppression by starving rapidly dividing cells of folate. Daily folic acid, usually skipping the methotrexate day, cuts those effects without erasing the benefit. High-dose oncology protocols use leucovorin rescue instead: same idea, bigger stakes.

Labs to monitor

CBC for marrow suppression, LFTs for hepatotoxicity, creatinine because the drug clears renally and accumulates in kidney injury. Baseline, then periodically per protocol. Teach the patient to limit alcohol: it stacks liver injury on a hepatotoxic drug.

Infection warnings

Immunosuppression means fever, sore throat, unusual bruising, or mouth ulcers get reported, not waited out. Live vaccines are avoided during therapy. Watch too for a dry cough or new dyspnea: methotrexate pneumonitis is uncommon but tested.

Pregnancy

Teratogenic and abortifacient. Contraindicated in pregnancy, and it is also the drug that ends ectopic pregnancies, which tells you what it does to a developing embryo. Reliable contraception during therapy and a washout period after stopping applies to both partners; verify pregnancy status before starting. NSAIDs and some antibiotics (trimethoprim) reduce clearance and raise toxicity: review the med list.

Frequently asked

Patient says they take methotrexate every day for RA. Reaction?

Stop and clarify immediately. Autoimmune dosing is weekly. Daily dosing is a life-threatening error.

Why prescribe folic acid with a folate antagonist?

It protects normal fast-dividing cells (gut, mouth, marrow) from folate starvation while the anti-inflammatory effect persists.

Which complaint worries you most on methotrexate?

Fever with a sore throat. It can mean neutropenia. CBC now, not at the next scheduled draw.
