Sildenafil and Nitrates on the NCLEX

The PDE5-nitrate interaction: why the combination is lethal, the 24 and 48 hour windows, and the questions to ask.

PDE5 inhibitors plus nitrates cause profound, refractory hypotension. This is the drug interaction the NCLEX tests most.

Why the combination is dangerous

Both drug classes raise vascular levels of the same messenger, cGMP. Nitrates generate it; PDE5 inhibitors (sildenafil, tadalafil, vardenafil) block the enzyme that breaks it down. Stacked, the vasodilation multiplies: blood pressure collapses, and the drop resists fluids and pressors because the mechanism is enzymatic, not volume.

The timing windows

No nitrates within 24 hours of sildenafil or vardenafil. No nitrates within 48 hours of tadalafil, whose long half-life earns the longer window. The reverse also holds: a patient on scheduled nitrates (isosorbide, a nitroglycerin patch) should not take a PDE5 inhibitor at all.

The chest pain scenario

The classic question: chest pain in the ED or on your unit, nitroglycerin due. Before the first sublingual tab, ask every patient about PDE5 inhibitor use in the past 24-48 hours. Patients will not volunteer it. Ask directly, and ask men and women both: sildenafil also treats pulmonary hypertension, and tadalafil treats BPH.

Who asks about what

The nurse giving nitroglycerin asks about PDE5 use. The nurse teaching a new nitrate prescription warns against PDE5 drugs. The patient starting sildenafil is asked about nitrate use, including as-needed sublingual tabs they may not consider a 'daily med'. Same interaction, three directions; the exam rotates which seat you sit in.

If the combination already happened

Hold all nitrates. Supine positioning, IV fluids, and hemodynamic support with close monitoring; the hypotension can be prolonged and severe. The tested nursing action, though, is the one that prevents it: the question you asked before the tab went under the tongue.

Frequently asked

Patient took sildenafil last night, now has chest pain. Nitroglycerin?

Hold it and tell the provider. Sildenafil within 24 hours contraindicates nitrates; the team treats the chest pain another way.

Why is tadalafil's window 48 hours instead of 24?

Longer half-life. The drug is still active a day and a half later.

Patient on a nitroglycerin patch asks about starting tadalafil for BPH. Answer?

The two cannot be combined. The provider must weigh alternatives; the patch does not simply come off for a day.
